Orthopedics Hawaii is not participating with all Quest and Medicaid Plans. Please be sure to obtain an authorization or referral from your health insurance prior to scheduling an appointment.

Health plans that require an authorization or referral include but are not limited to HMO plans (e.g. HMSA HMO, Kaiser HMO, EPO plans, etc.). Please include a scanned image of your health plan’s authorization or referral approval form when scheduling your appointment.

We recommend our patients review their health insurance policy (e.g. co-insurance/copayment amount, deductible, etc.). Please contact your health insurance directly for benefit details.

After providing your insurance, an out-of-pocket estimate can instantly be provided based upon your benefits and yearly deductible. Payment will be requested at check-in.
